For hotel reservation at the conference rate, please fill out the hotel reservation form .docx or .pdf and email it to islpedhotel@pku.edu.cn. The deadline is August 8, 2013.

Please do not contact the hotel directly.

In addition to the hotels at the conference rate, there are also other hotels in the area. If you choose one of the other hotels, please do not use this form.

On the hotel reservation form, please provide

2. First Name
3. Surname
4. Gender
5. City
6. Nationality
7. Date of birth (Year/Month/Day)
8. Passport Number
9. Arrival Date and Check-out Date
10. Choice of Hotel Name, Room Type (price), number of Rooms, Adults, Children.
